Findings: Witness #2 had previous known issues with the Alleged Victim's (AV) spouse, Witness #3 (W3). On or about August 26, 2021, W2 went to AV's room and yelled at AV because he/she was upset with AV's spouse. AV was intimidated and afraid of W2. The facility failed to properly care plan for the issues between W2 and W3, resulting in W2 yelling at AV, causing unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03253 $188.00 fine assessed
