Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about August 15, 2021, AV was exhibiting a physical decline. Between August 23, 2021, and August 25, 2021, AV suffered several skin injuries. The facility failed to respond to AV's significant change of condition, which is a violation of resident rights, is neglect of care which constitutes abuse.
