Findings: On or about August 27, 2021, it was discovered that the Alleged Victim (AV) was missing a card of narcotic medication. It was signed out as discontinued, with a forged signature of the facility Nurse. AV's medications were stolen by and unknown Alleged Perpetrator #3 (AP3), which placed AV at risk for harm. AP3’s (Unknown) actions are considered neglect of care and constitutes financial exploitation. The facility failed to provide a safe medication system and to protect AV from financial exploitation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
