Findings: Alleged Victim (AV) relies on the facility for his/her care and suffers from several existing skin integrity issues and skin breakdowns. On or about August 25, 2021, AV’s doctor wrote an order for AV to have a bed bath three (3) times a week and to receive wound care. As of August 31, 2021, AV had not had regular skin/wound care since moving to the facility on August 18, 2021. AV has had wound care 2-3 times since admission and only 1 bed bath. The facility failed to provide service to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
