Findings: Alleged Victim (AV) relies on the facility for care. On or about August 28, 2021, AV and Witness 1 (W1) were having an argument and W1 hit AV. Prior to being admitted into the facility, AV’s family let the facility know about AV and W1 having a history of altercations. The facility failed to properly plan care to prevent AV and W1 from being involved in a resident-to-resident altercations, which is a violation of resident rights, is neglect of care and constitutes abuse.
