- Findings
- Witness 1 (W1) frequently wanders into other residents' rooms. W1 has had multiple resident to resident altercations since June 25, 2021, almost all related to W1 going into other residents' rooms. As W1's wandering was thought to be related to W1's seeking a bathroom, W1 is service planned for staff to take W1 to the bathroom in the morning, before & after meals & before bed. Other interventions to include asking W1 to take a walk with staff, offering a snack, distracting with a preferred activity. On or about August 29, 2021 W1 was walking down the hall carrying a personal item W1 took from Alleged Victim (AV's) room & AV followed W1 & tried to get the item back from. W1 kicked AV in the shin, resulting in a 6" laceration of the shin. AV was required to go to a staff station to get assistance for AV's bleeding shin. The facility with the assistance of EMS provided wound care & bandaged AV shin. There is nothing in the documentation reviewed indicating that staff attempted service plan intervention s for W1's wandering into other residents' rooms. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
