Findings: Witness 1 (W1) frequently wanders into other residents' rooms. As W1's wandering was thought to be related to W1's seeking a bathroom, W1 is service planned for staff to take W1 to the bathroom in the morning, before & after meals & before bed. W1's Service Plan also indicates other interventions to include asking W1 to take a walk with staff, offering a snack, distracting with a preferred activity. W1 has had multiple resident to resident altercations since June 25, 2021, almost all related to W1 going into other residents' rooms. AV & W1 were seen arguing in the hall on or about August 28, 2021, about a stack of papers. W1 reached out & slapped AV & AV started yelling. Staff separated AV and W1. There is no documentation indicating that staff attempted service plan interventions for W1's wandering into other residents' rooms. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
Sanction: RCFCP25-00115 $375.00 fine assessed
