Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have showers twice a week. On or about July 26, 2021 through August 23, 2021 AV did not receive showers. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
