Findings: The Alleged Victim relies on the facility for his or her safety. On or about August 31, 2021 around 10:35am the facility received a telephone call, AV had been found approximately three blocks away from the facility. Camera footage shows AV leaving out the back patio door around 9:23am. It is believed the back gate was left open by the landscaping company. The facility failed to provide a safe environment, which is a violation of residents rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03617 $188.00 fine assessed
