Findings: Alleged Victim (AV) relies on the facility for his her needs. AV requires assistance with all ADLs, and requires hands on assistance to eat. The facility did not have sufficient staffing to meet AV’s care needs. Facility did not provide AV with necessary assistance to eat, leading to weight loss and serious loss of personal dignity which is a violation of resident rights, is considered neglect of care and constitutes abuse.
