Findings: Alleged Victim (AV) relies on the facility for his/her care. Witness #4 (W4) care plan states he/she is to be in the line of sight of Staff while out of his/her apartment. On or about August 23, 2021 W4 entered AV’s room and wheeled AV into the hallway and proceeded to hit AV. The facility failed to follow the care plan, placing AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03173 $500.00 fine assessed
