Findings: The Alleged Victim (AV) has a history of exit seeking. On or about September 6, 2021, AV left the facility unassisted and was found by security staff attempting to enter a vehicle. The facility failed to put interventions in place, which placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
