Findings: The facility failed to provide the basic care or services necessary to maintain Alleged Victim's (AV) health and safety which resulted in physical harm to AV. On or about September 9, 2021, AV was out in the community and got off the bus with assistance by someone who was not a facility staff member. AV suffered a fall, was sent to the hospital for treatment, and was diagnosed with a fractured left hip. AV requires a one-person assist with transfers prior to ambulating and stand-by assistance for ambulating. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
