Findings: On or about September 10, 2021, an Unknown Alleged Perpetrator 2 (AP2) left Alleged Victim (AV) unattended on the toilet while assisting AV with his/her toileting care needs. An investigation determined that AP2 left AV on the toilet and also left AV's apartment not to be found. As result of AP2 leaving AV unattended, AV attempted to transfer himself/herself from the toilet to his/her wheelchair which resulted in AV falling with injury. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care which constitutes abuse.
Sanction: ALFCP22-00283 $375.00 fine assessed
