Findings: Alleged Victim (AV) service plan indicates AV was an elopement risk and had steps for staff to follow to ensure AV was safe. On or about September 09, 2021, Alleged Perpetrator 2 (AP2) did not ensure the door was shut prior to opening the next, resulting in AV eloping from the facility without staff’s knowledge. AV was found in the parking lot picking up leaves. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative Rules.
