Findings: The facility failed to follow the care plan and provide appropriate services according to the Alleged Victim (AV)'s needs relating to ambulation. AV is a high fall risk and is care planned for staff to encourage AV to use a walker if AV is not using a walker upon every encounter. On or about September 10, 2022, AV was using a wheelchair for mobility, AV stood up and walked by staff w/o any mobility device. Staff did not intervene to assist AV as AV entered h/h room. Staff heard AV yelling and found AV on the floor. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00177 $375.00 fine assessed
