Findings: In August of 2021, the Alleged Victim (AV) was prescribed a medication for mood stabilization and aggression. On or about September 1, 2021, the AV ran out of his/her medication. The medication was reordered, however, staff that completed the re-order used the wrong health record and the medication was not refilled. On or about September 8, 2021, the medication was re-ordered, this time with the correct information and the re-fill was completed and medication received on September 12, 2021. AV went without his/her medication for almost two weeks, placing AV at risk for harm. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
