Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 11, 2021, AV was getting up from h/h chair, when Witness 1 (W1) walked up and hit AV in the head with a book. The facility failed to follow W1 care plan, interim service plan and behavior/safety plan for line of sight and 1:1 as active intervention for W1 behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00257 $500.00 fine assessed
