Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 16, 2021, Alleged Perpetrator 2 (AP2) had freely admitted to facility administrator and other staff that AP2 did leave AV on the toilet for two or more hours at a time. AP2 admitted that AP2 left AV on the toilet even though AV was asking to get off. On a separate occasion, AP2 was asked to assist with transfer of AV to bed and AP2 told Witness 3 (W3) to get out of the way, then proceeded to roughly pick AV up and “toss” AV onto AV’s bed causing AV enough pain that W3 administered PRN pain medication. AP2’s actions are considered physical abuse. the facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
