Findings: Witness #1 (W1) has history of going into other resident rooms, aggressive behaviors, and resident to resident altercations. W1 is care planned for staff redirection, 1:1 monitoring and walking with W1 outside. On or about September 14, 2021, W1 entered Alleged Victim (AV) room causing AV to yell at W1. W1 hit AV in the nose. The facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: ALFCP22-00380 $250.00 fine assessed
