Findings: Alleged Victim’s (AV’s) service plan dated March 1, 2021, notes that AV is not a fall risk and needs no assistance with ambulation and transfers. AV was noted to have fallen six (6) times in less than three (3) months; on July 1st, 12th, August 12th, September 9th, 11th and 13th, 2021. The fall on July 12, 2021, resulted in shoulder pain. The fall on August 12, 2021, resulted in scrapes to AV’s face, neck and arm. The fall on September 9, 2021, resulted in a bump on AV’s head and the fall on September 11, 2021, resulted in increased facial swelling and bruising and a skin tear to AV’s arm. As a result of the falls from July 1st and 12th, 2021, AV was sent to the emergency room. The facility failed to care plan and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
