Findings: Alleged Victim (AV) relies on the facility for his/her care. AV requires full assist with two showers weekly, however, the facility did not provide two showers weekly to AV. Staff are taking long to respond to the call light. Between August 1st, 2021, through September 23rd, 2021, there were 37 responses that took more than 14 minutes and 21 that took over 30 minutes. The facility failed to provide service, which is a violation of resident rights, is neglect of care and constitutes abuse.
