Findings: Alleged Victim (AV) has a known history of falls. Between August 7th through September 14, 2021, AV sustained three falls and no precautions or interventions were put in place following those three falls. On or about September 19, 2021, AV had an unwitnessed fall in his/her apartment. AV was found on the ground and in pain. AV was transported to the hospital and was diagnose with a T12 compression fracture. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP22-00518 $1500.00 fine assessed
