Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history of three (3) documented falls occurring on or about July 02, 2021, through July 08, 2021. On July 08, 2021, AV was found on the floor that required a trip to the Emergency room as AV reported “lots of pain”. The facility failed to appropriately care plan and implement reasonable interventions to address Av's falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
