Findings: Alleged Victim (AV) has history of wandering the facility. On or about September 23, 2021 Witness 1 (W1) was sitting at the dining room table and saw AV entering another residents room. W1 got up from the table and confronted AV. The altercation escalated, W1 slapped AV across the face and AV punched W1. There were no staff in the vicinity of the dining area where residents were placed for lunch. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s wandering into other residents room, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
