Findings: Alleged Victim (AV) relies on the facility for a safe environment. On or about June 12, 2021 AV had a sexual encounter with Witness #1 (W1). W1 has history of sexual encounters with different residents. W1 did not have a Safety plan in place until approximately June 24, 2021. The facility failed to provide a safe environment and properly care plan which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03288 $1500.00 fine assessed
