Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 23, 2021, Witness 1 (W1), was showing aggressive behaviors. W1 came up to AV and hit him/her in the back of AV’s legs with their walker. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
