Findings: The facility failed to follow Alleged Victim (AV)'s care plan. On or about September 25, 2021, AVs interim service plan notes, staff to toilet h/h at the beginning, middle and end of each shift. Documentation provided to the Department indicates On or about September 27, 2021, AV was found to have a hematoma and scratch to h/h right forehead from unknown cause, resulting in AV being sent to the hospital for evaluation. AV was observed sleeping at approximately 10:47am, and was not checked on at change of shift, the facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00510 $1125.00 fine assessed
