Findings: Alleged Victim (AV) relies on the facility to administer his/her medications. On or about September 27, 2021, Alleged Perpetrator 2 (AP2), was using a parameter note that was noa approved by the facility. AP2 administer 40 units of AV’s medication versus that is should have been administer, which is 4 units per sliding scale parameters. AP2’s failed to provide a safe medication administration system to AV. The facility failed to provide a safe medication administration system for AV, which is a violation of Oregon Administrative Rules.
