Findings: On or about September 25, 2021, Alleged Victim (AV) left facility without assistance and without injury. AV was found in the community by law enforcement experiencing confusion and was transported back to the facility by law enforcement. An investigation determined that the facility failed to put interventions in place to address AV's increased confusion, leading to AV being lost and confused in the community on September 25, 2021. The facility's failure to properly care plan for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00300 $375.00 fine assessed
