Findings: On or about April 10, 2021, Witness #2 (W2) was walking by the Alleged Victim's (AV) room and thought he/she heard the knob jiggling, and 10 minutes later, W2 was going to AV's room to give AV his/her medications and found the door locked. W2 opened the door to find AV inside, looking terrified and the room was completely dark. W2 asked Alleged Perpetrator #2 (AP2) why AV's door was locked and AP2 replied that AP2 locked AV in his/her room because he/she was pacing too much. AP2's actions are a violation of resident rights, are considered neglect of care and constitute involuntary seclusion. The facility failed to protect AV from involuntary seclusion, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
