Findings: The Alleged Victim (AV) had a change of condition, and his/her care plan was updated after his/her change of condition, however, it was not updated after he/she began to have falls. AV's care plan was not sufficient to mitigate AV's risk of falls. The facility's failure to properly care plan left AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
