Findings: On or about September 29, 2021, Alleged Victim (AV) was sitting at the dining room table when Witness 1 (W1) walked up and hit him/her in the head with a plastic serving tray. W1 Service plan indicates Staff to have visual contact with W1 when she/he is up and wandering. Facility failed to follow W1 care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00319 $500.00 fine assessed
