Findings: Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) had a known history of resident-to-resident altercations. On or about September 30, 2021, W1 punched AV in the arm. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behavior, which is a violation of resident rights, is neglect of care and constitutes abuse.
