Findings: On October 1, 2021, Alleged Victim (AV) left the facility unattended and without injury. An investigation determined that AV showed signs of confusion and while in community walked to the police station. An investigation determined that facility failed to provide sufficient staff to follow AV's temporary service plan put into place on September 30, 2021, to check for AV's location every thirty minutes and by failing to promptly call the police when it was discovered AV was missing. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
