Findings: Witness 1 (W1) has a known history of attempting to or having bitten staff and or residents. On or about October 2, 2021, W1 bit Alleged Victim (AV) on his/her right wrist. W1 had previously bitten AV on his/her left hand the day prior. AV did get bite wounds/scabs from those bites. The facility failed to provide AV a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00845 $250.00 fine assessed
