Findings: On or about October 3, 2021, the Alleged Victim (AV) was sitting in the hallway with his/her head down around his/her knees, holding on to the chair next to him/her. Alleged Perpetrator #2 (AP2) was seen on video placing his/her hand on AV's forehead pushing it back and speaking to AV in an abrupt tone. AP2's actions are a violation of residents rights, are considered neglect of care and constitute physical abuse. The facility's failure to protect AV from physical abuse is a violation of Oregon Administrative Rules.
