Findings: The Alleged Victim (AV) is known to have skin issues and the facility is to monitor these issues. Home Health had requested for barrier cream to be applied twice daily to the area for prevention. The facility failed to place this information in AV's care plan, therefore, the barrier cream was not being applied as ordered. AV went to the hospital on October 1, 2021 with a stage 2 pressure ulcer. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
