Findings: The Alleged Victim (AV) is very high functioning and is known to have the ability to learn and retain the code to open the door of the memory care unit. AV's service plan states that staff are to be aware AV was able to learn and retain the code and were cautioned to ensure they were blocking the code when AV was present. On or about October 1, 2021, AV used the code and exited the unit with another resident. AV and the other resident were found approximately 30 minutes after exiting the building. Neither resident was harmed, however, both residents were placed at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
