Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV suffered multiple falls within the month of August, injury and non injury, no meaningful interventions were put into place until the end of August to try to reduce the amount of falls for AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
