Findings: On or about October 5, 2021, Alleged Perpetrator #2 (AP2) buckled the Alleged Victim (AV) in his/her wheelchair in order to continue cares for other residents. AV is not care planned to be buckled into his/her wheelchair, and buckling a resident to a wheelchair is a restraint. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failed to ensure AV's rights, which is a violation of Oregon Administrative Rules.
