Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 31, 2021, AV suffered a fall and was identified as a fall risk. AV had several additional falls between August 2, 2021, and August 12, 2021, where AV experienced head injuries and bruises. The facility failed to implement interventions to mitigate the risk of injury due to AV's increase in falls. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
