Findings: On or about October 6, 2021, the Alleged Victim (AV) was found to have a bruise on his/her forearm. AV had complained of rough care by staff members. AV's care plan does not address AV's combative behavior, although staff are directed to step away and re-approach when AV is being resistant to care. The facility failed to properly care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
