Findings: The Alleged Victim (AV) was a known fall risk and had 6 prior falls. On or about October 4, 2021, AV had a fall turning from the kitchen counter, was transported to the hospital where he/she was diagnosed with a broken arm between the shoulder and elbow and received 4 stitches in his/her forehead for a laceration. The facility failed to properly care plan or put meaningful interventions in place to reduce the amount of falls for AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
