Findings: Alleged Victim (AV) relies on the facility for his/her care. Between August 28, 2020, through October 6, 2021, AV sustained six falls resulting in injuries to his/her head and bruising. On or about October 6, 2021, at about 11:45 pm AV fell in his/her room. AV sustained a skin tear on his/her left forearm as well as a fractured femur. There were no new fall prevention interventions noted for any of the falls. The facility failed to care plan appropriate and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
