Findings: The facility failed to provide basic care to Alleged Victim (AV) which resulted in a risk of harm. An investigation determined that AV did not receive assistance with his/her peri-care on several days throughout the month of September 2021 (three days on evening shifts) and several days in the month of October 2021 (six days at various shifts). The facility's failure to follow AV's care plan for peri-care is a violation of resident rights, is considered neglect of care and constitutes abuse.
