Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about October 12,2021, AV eloped from the facility through an unsecured gate on the patio. AV was located at the apartment building across the street with cold hands. It is believed the landscapers left the gate unlocked. AV’s service plan has AV noted for wandering. The facility failed to provide a safe environment resulting in risk of serious harm to AV, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00334 $375.00 fine assessed
