Findings: Alleged Victim (AV) has a known history of falls. Between September 27th, through October 12, 2021, AV sustained approximately five (5) falls, some resulting in injury. On or about October 9, 2021, AV fell in the dining room and the intervention was to have care staff in the dining room when AV is present. On or about October 11, 2021, AV again fell in the dining room, staff were not present and again the interventions were to have staff present in the dining room. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00901 $1500.00 fine assessed
