Findings: Witness #1 (W1) has a history of aggressive behavior with other residents. W1 behaviors escalated and resulted in resident-to-resident altercations on or about September 03, 2021, October 08, 2021, and again on October 13, 2021. The facility failed to appropriately care plan and implement reasonable interventions to address W1 ongoing behaviors, which is violation of resident rights, is considered neglect of care and constitutes abuse.
