Findings: On or about October 10, 2021, Alleged Victim (AV) was found unkept with poor hygiene. An investigation determined that AV was care planned to have a shower two times per week but facility documentation showed AV did not receive showers according to AV's care plan. It was determined that AV was often resistant to care and would become physically aggressive. The facility’s failure to properly care plan for AV's behavior resistance to care is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-00326 $250.00 fine assessed
